An investigation into smudging in �General Mehmandarov’ vessel of the Azerbaijan Caspian Shipping Company (CSC), which is under the refurbishment at the Caspian ship refurbishment Production Association. The investigation revealed that the reasons for the incident were negligence by the employees of the hull-welding shop and members of the crew.
"The commission established in this connection revealed that on 19 August at 07:10 three employees of the hull-welding shop without warning the crew and permission started welding work in board of �General Mehmandarov’ vessel. At this time oily rags, left without control, fired and resulted in wreck of the isolation of electricity transmission lines. The crew did not carry out safety measures during the repair of the vessel and did not observe the safety instructions in the machine-room.
At the order of the chief of the CSC wages of 3 employees of
hull-welding shop and they were issued strict reprimand. Besides, 4 members of the crew, including captain and senior mechanic, were lowered in rank.
