"Significant errors and misjudgement" within the oil industry and US government led to the BP oil spill in the Gulf of Mexico last year, the co-chairman of the committee investigating the disaster said Tuesday, dpa reported.

The National Commission on BP Deepwater Horizon issued its final report that concluded "systematic failures" took place by BP and its drilling partners in the gulf as well as by US government regulators, former senator Bob Graham said.

"We concluded that these mistakes amounted to a significant failure of management," Graham said at a press conference in Washington.

Millions of gallons of oil poured into the Gulf of Mexico in the months following the April 20 explosion on a rig leased by BP from Deepwater Horizon. Eleven rig workers died in the blast.
