The Material Girl star, 49, was photographed by Steven Klein for the April 2008 issue of the publication, which is on newsstands in the States now.
It is the last issue to be edited by Ingrid Sischy - who also quizzed Madge for the cover piece.
The singer tells the magazine about her work in Malawi, and says "there's a sense of urgency in everything I do".
The cover photo shows the Queen of Pop, dressed in little other than a tight black leotard, boxing boots and gloves, and posing provactively.
The magazine interview comes days after she wore a see through dress at her induction into the Rock and Roll Hall of Fame in New York.
The singer who will be 50 this year, shows no sign of covering up her figure, or slowing down with the intensive gym workouts she undergoes on a daily basis to maintain her muscular physique.
